PPS 2015 Mystery Case Answers:

1. Domestically acquired particulate lung disease (hut lung) with progressive massive fibrosis
2. Chronic hypersensitivity pneumonitis mimicking UIP with positive exposure history to feather pillows and confirmatory HSP panel.
3. Aluminum pneumoconiosis  
4. Desqaumative interstitial pneumonia.  Some debate about whether there was associated acute lung injury and/or whether this was alveolar hemorrhage--discussion unresolved. 
5. Diffuse interstitial amyloid of ATTR type and sarcoidosis involving lung; amyloid and sarcoidosis also involving heart.  
6. Anti-Jo1 related ILD.  
7. Idiopathic pleurparenchymal fibroelastosis.  
8. ILD related to Hermansky-Pudlak syndrome.  
9. Multiple minute meningothelioid nodules/meningotheliomatosis.  
10. Idiopathic pulmonary hemosiderosis with iron encrustation of vascular elastic fibers.   Some debate about whether this was truly idiopathic or autoimmune.
